My mother is taking warfarin 6mg and INR value is 3.35, test 36.5 control 11.9 form today blood report. 15days ago INR Value was 1.88, test was 22.4 and she was taking 5mg warfarin. so what dose of warfarin should she take now? she had heart valve replacement surgery few month back.

Target INR for Mechanical Valve replacement is 2.5-3.5! I suggest continue taking 6 mg and follow up PT INR after 2 wks!
